Office Policies

Insurance & Payments

Payments are due at the time of service.

If you are covered by an Insurance Plan that is “IN Network” with Sugarloaf Pediatrics, we will gladly process them for payments for the care provided. You must come prepared with you Insurance Card at the time of each visit, especially at the time of the first visit. Also, please be assured that Sugarloaf Pediatrics is listed as the patient’s Primary Care Provider prior to the appointment.  In the event that you have co-pay or a deductible, or if the patient receives any services that are not covered by the Insurance, patient is still responsible for payment of those services at the time of the Office Visit.  If you are under an Insurance Plan of which our clinic is not ” IN Network” with or no insurance,  still wish to be seen, our front desk team can provide you with our Self Pay rates. 

If you ever have any questions in regards to your account, balances, bills, or insurance coverage, please contact our front desk and they will do their best to answer any questions.

Records Transfer 

When requesting Medical Records to be transferred to another physician’s office or clinic, we will need a Medical Release Form signed by the patient’s parent or guardian (if the child is under the age of 18). Once the forms are received, we will send out the records. In the event that you are requesting copies of Medical Records for personal use, there will be a fee of $30 per record.

School Forms 

• Sports Physical Forms

To have a Sports Physical Form completed for the child, you will need to drop off the form to the clinic with the “History Form” portion already completed. However, if the patient DOES NOT have an up to date Physical Exam, you will be required to schedule an appointment to get the child up to date before the physicians can sign off on the Sports Physical Form. The turnaround time for Sports Physicals will always be 24 hours, and will need to be picked up once completed. 

• 3231 & 3300 Forms

Granted that the patient has an up to date physical and is caught up on immunizations, the 3231 & 3300 Forms can be picked up in office and will require a payment of $5 per form. To avoid charge you may request these forms at the time of the patient’s yearly physical, and we will gladly provide you with them.

Vaccination Policy

Sugarloaf Pediatrics is a private practice, and our providers have decided that we only accept patients who will be receiving all the vaccinations as recommended by the American Academy of Pediatrics. This does not include optional vaccines such as the Flu & COVID-19 vaccine.  

Appointments & Cancellations

We kindly ask that all patients honor their scheduled time for their reserved appointments. If you ever are unable to keep an appointment, please notify us as soon as possible. Failure to notify the office of a missed or cancelled appointment in a timely manner will result in a no show fee. 

After Hours & Emergencies

Emergencies

If you are experiencing a life threatening medical emergency, please call  911 immediately.  If your child needs to be seen by a physician after Office Hours, please take the child to any Urgent Care Center such as the Children’s Healthcare of Atlanta at Satellite Boulevard or to the Pediatric ER of Northside Hospital of  Gwinnett  or any other Hospital.

After Hours

We have an After Hours call service available that is to be used for urgent situations . You may call (678) 377-1113 to reach our Interactive messaging. You may leave the call back number.  We kindly ask that you do not use our after hours line for questions regarding appointment scheduling, insurance, or referrals.

For your convenience, one of our physicians will always be on call twenty-four hours a day, seven days a week. If you are calling after hours, they will be available to give advice for emergency medical concerns; however, our doctors will not prescribe medications after hours unless it is necessary. 

If you are asked to leave a message, please provide the following information in the message:

  • Patient’s name
  • Patient’s date of birth
  • The reason for the call
  • A good phone number to reach you
